Home is Where the Heart is Support Worker 

Home is Where the Heart is our residential programme that provides placements for young people aged 16 to 23 years; who are moving on from the care system or for young people who have for whatever reason found themselves with nowhere to live 

Home is Where the Heart is, is a programme that is tailored to meet the needs of each young person, offering intensive 1:1 support to help young people to transition into adulthood. Each young person will follow an independent living programme which is designed to ensure that young people are equipped with the skills they need to live independently. 

As well residential support, the Home is Where the Heart is team offer an outreach service to support to young people who are in care and are close to the point of moving to independent living as well as supporting young people who are already living independently. 

In our delivery we aim to help young people to develop their resilience, confidence and emotional coping strategies as well as developing the skills they need in order to live independently. 

Young people referred to HIWTHI will have full access to EITC programmes which they will be able to access if it is beneficial to them. 

Key Responsibilities  

  • Support the HIWTHI Manager and Co-ordinator to ensure that young people are recruited, developed, engaged and monitored in line with Everton in the Community guidelines. 

  • Staff should have a full understanding of the project and the policies, procedure and regulations that govern it. 

  • To work with a small case load of residential placements and engage them in the HIWTHI programme. 

  • To work with a small but intensive case load of out reach young people, aged 16-23, at education, home addresses and care facilities; to build trusting and professional relationships and engage them with HIWTHI and in turn the wider EITC offer. 

  • Create a comprehensive risk assessment to ensure the safety of all staff and young people resident at the home. This must fall in line with EITC policies and procedures.  

  • Assess the needs of the young people and create a tailored care and action plan. Taking into account the choices, needs and wishes of the young people, the young person should be involved in the writing of the plan and they should be encouraged and supported to work toward the goals set out in it. 

  • Take personal responsibility for learning about the young people and in particular any difficulties that may impact on their progress. Staff must take time to listen to the young people and ensure that their views and wishes are considered. 

  • Staff should encourage young people to take responsibility for their mental health, staff should support them when needed to ensure that they are able to access the right support if it is needed. 

Ideally we are looking for someone who has experience of working with young offenders, young people at risk of offending or individuals displaying challenging behaviours. You will also need experience of 1-1 support work with young people, this should include assessment, intervention planning and mutual care plans.

Please note this is a fixed term contract, and will be shift work with weekends included.
